All cases shall be documented as to reason for referral, services provided [i.e., assistance with appointments at Military Treatment Facility (MTF); VA health facilities; civilian health care provider; chaplain; other), follow-up contacts with member, and status/resolution of psychological health and/or Traumatic Brain Injury related issues.
